v0.4.18: Domain listing & filtering (Issue #7)

Adds domain discovery and filtering for targeted pattern retrieval.

### Added
- `/domains` command: Lists available domains with pattern counts
  - Groups by size (Major/Medium/Small)
  - Shows domain names for filtering
- Domain-filtered search:
  - `--allowed-domains` (whitelist)
  - `--blocked-domains` (blacklist)
- `ace_search` tool: `allowed_domains` and `blocked_domains` parameters
- `/status` now shows top 5 domains and helpful/harmful totals
- Domain-aware workflow in instructions and SKILL.md
