• Meta Model API provider (meta/*): OpenAI Responses API against
  api.meta.ai/v1, key via META_API_SK, requests sent with
  store: false. Effort is sent as reasoning.effort. Muse model ids
  resolve to creator meta.

Changed

• An output effort is accepted only when the entry's thinkingEffortLevels
  declares it; none is no longer accepted on every model.
• An explicit outputEffort on the envelope is validated like the :effort
  suffix, and fails with SESSION_INVALID_OUTPUT_EFFORT.
