Incognito toggle for Codex desktop.

Changelog

1. Host-based macOS launcher icon: The Finder launcher now uses the host's current system icon with an INCODEX glass plaque, keeping the ChatGPT or Codex mark without copying or modifying the official app. Icon Composer is not required on users' machines.
2. Existing launcher compatibility: Recognized older entries can be upgraded, and an owned entry remains removable even if its source host is gone. Failed icon generation preserves the previous entry.
3. Illustrated manual: The bilingual launcher guide now includes the icon design, size comparison and native-generated output, with previews clearly distinguished from installed Finder screenshots.

Upgrade with inc update, then run incodex launch to regenerate the Finder entry. This icon change does not require re-signing or reinstalling the host. Runtime behavior and existing platform limits remain unchanged.

Known verification gap: automated checks cover composition, image orientation, color profiles, migration and removal, but end-to-end display and clicking of the installed Finder entry have not yet been accepted. This release ships with that gap disclosed.

2. 兼容已有入口：已识别的旧入口可升级；即使源宿主已不存在，自有入口仍可移除。图标生成失败时保留原入口。
3. 手册加入图示：双语启动器指南新增图标设计、尺寸对照和原生生成结果，并明确区分预览与已安装 Finder 入口截图。

运行 inc update 升级，再运行 incodex launch 重新生成 Finder 入口。本次图标改动不需要重签或重装宿主。Runtime 行为与现有平台限制保持不变。

已知验收缺口：自动检查覆盖合成、图像方向、色彩配置、迁移和移除，但已安装 Finder 入口的实际显示与点击尚未完成验收。本版如实保留这项缺口发布。
