Session and lifecycle fixes for current dsh releases.

• Nudge reminders now use an identified user message with a producer-owned source kind, so durable session encoding and resume no longer fail after a reminder is injected.
• session/flush is treated as a durability checkpoint, not session end; the session-end derive hook runs on session/disposed, and both session / session_end trigger spellings are honored.
• Plugin reload/HMR now disposes nudge and inject listeners instead of leaking them.
• Injected memory/SOUL text opts out of dsh prompt-variable interpolation, so literal {{...}} cannot break prompt assembly.

Tarball includes compiled lib/, templates, migration script, CHANGELOG, and bundle patch. Upgrade in place; no data migration is required. See CHANGELOG.md for details.
