v1.8.1: fix: Correct form submission in entity create modal

Fixed a bug in the entity create modal where form submission was failing due to incorrect form element selection. Changed from selecting the entire modal element to selecting the form within the modal, and updated the fetch request to properly handle FormData instead of URLSearchParams.
