Open questions carry stable labels (Q3) written into the file, and the question tools address them by label: resolving or splitting one question never changes which question another label names. Splits are numbered Q3.1, Q3.2; decisions are recorded under their label; labels are never reused. Unlabelled questions are reported with provisional labels and labelled on the first question write or by fix. Breaking: resolve_question and split_question take id instead of index, next_question takes a label for after, and results report id.
