Fixed

• Reinstalls of active plugins (e.g. after a hub repack-from-workspace) now
  deactivate before reinstall and reactivate after. Plugin_Upgrader::install
  with overwrite_package was silently failing to replace files on active
  plugins, leaving the old version installed despite reporting success.
