Simplify example file structure

This makes the practice exercise's structure more in line with the concept exercise's structure

This seems to be changing the location of the example.clj to ".meta/example.clj"

The PRs that refactor test.clj and scaffold the exercises, still appear to be using "./meta/src/example.clj". Not sure about the PR that verifies the exercises, though.

This appears to fix the formatting in the generator files and it also gets rid of the "src" folder from everywhere.

There's only a minor issue.

The exercises/practice/list-ops/.meta/src/example_lists.clj file, which was my solution to the optional goal for this exercise is gone too :) Can you please create an "extra" folder and put it there?
