fix(service): shadow ledger append clobbered existing records below the 512KB cap

Existing lines were only re-read in the compaction branch, so any append to a
ledger smaller than 512KB rewrote the file with just the new entry (found by
the dsh-self-improve N1 wave's ledger cross-check). Now every append carries
existing lines forward; compaction still trims to the newest 3/4 past the cap.
Protects both package-shadow and lesson-shadow ledgers (shared helper).
Regression test pins two-entry survival.
