Across 569 observed runs in the 24-hour window ending 07:55 UTC, the most revealing contrast was between checks that proved a narrow path worked and adjacent workflows that still could not settle or publish their work. The day also showed a coordinated review fan-out: five distinct reviewers completed under one recorded command episode. The command's root run is absent from the selected artifacts, so its human trigger cannot be attributed.
Episode Highlights
The work queue passed a smoke check while its harder boundaries remained unsettled. An engine conformance worker failed claim reconciliation and Issue projection in §40083405475. The Smoke Work Queue Dispatcher then failed trusted controls and verified dispatch in §40102973967. Later, §40146211671 completed its agent, safe-output, verification, and conclusion jobs. Formal verification's two check jobs passed, but its analysis handoff upload failed in §40130187502. These are distinct boundaries; the evidence does not establish one cause.
Five reviewers moved together. A recorded
agentic_commands.ymlepisode grouped Design Decision Gate, Matt Pocock Skills Reviewer, Ponytail Reviewer, PR Code Quality Reviewer, and Test Quality Sentinel on the same branch. All five child runs succeeded; §40115223401 is a representative run. Their common episode ID is strong lineage evidence, although the parent run was outside the collected selected artifacts. The cross-repository PR smoke pair stalled before agents could act. Create and update paths both failed while checking out
github/gh-aw-side-repo: §40147033704 and §40148115535. The shared failed step is clear; the underlying checkout cause is still unknown. The safety signals split. The Safe Output Health Monitor failed its agent execution and conclusion in §40161732282, while the later Daily Safe Outputs Conformance Checker completed agent, safe-output, evaluation, and conclusion jobs in §40166597200. Separately, the container image scan updated several CVE burn-down issues, including reopening #52657, before its vendored vulnerability and license gate failed in §40165956449.
Feedback Loops Across Workflows
The Publication-boundary repair loop remains mixed: a later smoke success gives useful evidence, but conformance settlement, dispatcher verification, and formal handoff still need separate recovery. The Cross-repository smoke validation loop remains stalled at the same checkout boundary in two workflows. A new Monitoring and conformance divergence signal needs watching because a passing checker did not coincide with a passing health monitor.
Human Interventions That Mattered
Peli de Halleux merged PR #71833 at 06:41 UTC, changing how declared network credentials are handled in safe-output integration jobs. This was a concrete maintainer change during the window. It followed the monitor failure and the passing conformance run, so these observations do not prove a recovery from the merge. The five-reviewer fan-out has a recorded workflow command root, but the missing parent keeps its human initiator unverified.
Signals to Watch Next
Watch for a complete work-queue settlement after the smoke pass, a successful side-repository checkout in both PR smoke paths, the next health-monitor result after PR #71833, and a container scan that clears its gate while keeping the issue ledger current.
Coverage and interpretation
The cached collection had 325 later runs; the required continuation added 244 earlier runs, and the final cursor returned zero runs. The two batches cover 2026-10-24 08:23–18:00 and 18:21–2026-10-25 07:53 UTC. A 20-minute interval between batch endpoints had no returned runs. The review fan-out's parent is missing from selected artifacts; failed-step clusters describe symptoms, not proven causes. No intentional failure is used as a repair recommendation.
