Storify Daily Entry (2026-10-30)
In the 24 hours ending 2026-10-30 07:55 UTC, the downloaded index and its continuation covered 538 workflow runs. A repeated publication failure dominated the failed-run sample, while a smaller burst stopped several workflows before their agents could start. In the same window, a human-led change to protected-file exclusions moved through multiple agent reviews, an ADR gate, revision, and merge.
The contrast is useful: PR #72958 shows a feedback loop with a visible human decision and completed outcome. The failure clusters show recurring boundaries, but their causes and recoveries remain unproven.
Episode Highlights
Publication after successful agent work stalled. The same
safe_outputs / Process Safe Outputsfailed-step cluster contains 95 run IDs in the later index and six in the earlier continuation. Earlier examples span Matt Pocock Skills Reviewer, PR Code Quality Reviewer, and PR Sous Chef; the later representative is PR Sous Chef §41021354879. In the audited earlier §40861774464, the agent job succeeded and publication failed. This extends the prior day's publication-boundary pattern. Confidence is high in the boundary and recurrence, low in a shared cause. An activation prerequisite stopped four workflows. Four runs between 09:00 and 10:20 UTC share a failed
activation / Restore daily AIC scan observationsstep: [aw] Failure Investigator (6h), Glossary Maintainer, Issue Monster, and Copilot PR Conversation NLP Analysis. The audited §40856237978 failed in activation, so no agent outcome can be inferred from it. Confidence is high in the common step, low in whether the underlying failure was the same.A design gate changed a human-authored PR. Peli closed the narrower #72950 at 11:33 UTC and opened repository-level #72958 at 11:48. Design Decision Gate §40868959457 requested an ADR and committed a draft; reviewer workflows also completed. Peli then accepted four concrete review fixes, linked the ADR, and merged at 12:23. Confidence is high in this sequence; the data does not measure later production effects.
Feedback Loops Across Workflows
Publication-boundary repair loop — unresolved. Successful agent jobs repeatedly fed a failing publication step across multiple workflows. The pattern persisted from the previous window; no linked fix or recovery is established here. Cluster membership is a symptom count, not a root-cause count.
Review-gated design loop — improving in this episode. The gate exposed missing architecture documentation, reviewers found specific scope and glob issues, and the author revised and merged the PR. The visible improvement is the reviewed change reaching merge; runtime benefit remains to be checked.
Activation-prerequisite loop — direction unknown. Four workflows stopped at the same restore step. There is no evidenced human intervention or later recovery in this sample.
Human Interventions That Mattered
Peli's PR #72958 responses explicitly accepted a corrected portfolio assertion, collision-free glob conversion, and tighter exclusion schemas. The ADR link and merge followed the gate's request. This is the clearest observed human pivot; the publication and activation clusters have no comparable intervention tied to them.
Signals to Watch Next
Check the exact publication errors and whether a later PR Sous Chef or reviewer run clears the step. Check whether the activation restore step recurs after 10:20 UTC, and whether PR #72958's merged exclusions behave as intended in subsequent runs. The run index establishes coverage for the requested window, but these causal and post-merge questions remain open.
