⚠ BREAKING CHANGES

• sdk: QueryError, PermissionError and TransportError are
  gone. Branch on err.code and err.scope instead of err.name:
  PermissionError is code forbidden or unauthenticated, TransportError
  is scope server, QueryError is query_invalid or query_failed.
  ValidationError stays for declarations, with code
  declaration_invalid. A plugin refuses a malformed call with
  invalidRequest() rather than ValidationError.
