Simplified workspace structure

• All decisions now route to context/decisions/{slug}.md — previously split between context/decisions/ (via /draft:learn) and docs/decisions/ (via draft-executor). The inconsistency caused the agent to write to the wrong path. Now all decisions go to one place.
• docs/ is now a flat folder of written artifacts (analyses, PRDs, strategies) with YYYYMMDDHHMMSS_descriptive-slug.md naming. No subdirectories.
• Removed templates/ references from all agent definitions and skill files.
• workspace-template/context/decisions/ — scaffolded on fresh install.
• workspace-template/docs/ — scaffolded on fresh install.
