What changed
- Added a persistent Enabled switch to each provider in Settings.
- Hide disabled providers and providers without an installed agent from limits, spend totals, the menu bar and alerts.
- Detect installed CLI agents and desktop apps, and refresh availability when usage refreshes.
- Preserve saved history when providers are disabled and restore it when re-enabled.
- Preserve existing Cursor opt-outs on upgrade; provider switches are separate from remote API-check settings.
Validation: 120 tests passed, including provider persistence, installation changes, cached-data filtering and history restoration.
