This patch fixes getReadline not holding a reference to the TTY correctly. Now, it holds a reference count and only unref's the TTY when there are no holders. This also fixes the case of multiple getReadline calls within a given scope, since an inner disposal doesn't unref while the outer one still needs the readline.
