Fixed

• Clear removed handler references for handle-based unsubscribe, function-based
  unsubscribe, and completed Once subscriptions; remove empty subscriber entries
  so the bus no longer retains closures after their last registration is removed.

Changed

• Skip event wire-name computation for local publishing without observability,
  and measure handler duration only when observability is enabled.
• Remove unused internal handler metadata and the SQLite unbatched streaming path
  that was inaccessible through public options. ReadStream keeps its existing
  batched behavior, including cancellation, error reporting, and early termination.
• Make concurrent benchmarks publish exactly the reported number of events and
  separate dispatch from formatting/channel overhead. Replace a log-only timing
  test with a deterministic check that concurrent publishers can enter Append.
• Document SQLite batching, subscription-handle cleanup, optional instrumentation,
  and reproducible dispatch benchmarks in the guides and API comments.

Includes #55. No public API changes.
