Add /restart command and improve monitor reliability

- Add /restart Telegram command to restart ccc service
- Fix monitor not syncing after user messages by properly tracking
  session state (ResetSessionMonitor vs ClearSessionMonitor)
- Update polling schedule: 3s for first minute, 30s for next hour,
  then stop polling
- Clear monitor state on /continue, /new, /delete
