loadmaster doctor no longer asks a repository whose own commands go through loadmaster to add an instruction line.

Fixed

• doctor asks for no change that is not needed. A repository whose package.json, Makefile, justfile, Taskfile, mise.toml or deno.json runs its checks with loadmaster run, or guards them with loadmaster held, puts an agent that uses those commands in the queue without telling it. 0.16.0 still told such a repository to add a line to AGENTS.md, which an agent following doctor might do; now the instructions check is ok there, and says why.
