Reduce tool waits while chats work in parallel

Changes

• Group already-received tool results and routine progress updates before saving, reducing repeated work that can hold up file reads and app actions when several chats are active. Results keep their original order and are saved before the next command; permission checks, approvals and cancellation boundaries remain enforced.
• Refresh saved-chat metadata without repeatedly processing an unchanged workspace list. Changes to workspaces, availability, counts, read state and selected history still update normally.
