Core Runtime & CapabilityBus Unification (#114): Integrated OPCEngine and declarative opc-pack/v1 business packs into MekongCoreRuntimeImpl, CapabilityBus, and Governance.
2-Goal Human Approval Lifecycle (#114, #115):
- Goal 1: mekong opc proposal --wait-approval (and mekong run --pack client-proposal-followup) generates structured quotes and pauses in PENDING_APPROVAL with a canonical SHA-256 payload digest.
- Goal 2: mekong opc decide --run-id <id> --decision [approve|reject|revise] enforces run status checks, issues single-use tokens, atomically consumes approvals via SQLite WAL (consume_approval_atomic), reserves pre-dispatch budgets (BEGIN IMMEDIATE), and dispatches connectors.
