• 唤醒前清掉上一条已投递的唤醒行；每 30 秒清理已投递（inactive）的唤醒行；重载后首次 tick 连带清理历史遗留
• 只匹配 同 Session + 同标题 + kind=after + 非 active，不碰真实定时任务

• 新增 wakeOnChange.keyPattern：正则匹配到的片段作为「行身份」，同一会话正文/时间变化不算新条目，只有冒出新会话才唤醒
• 未配置 keyPattern 时行为不变（整行比较）
