Unbricked the mod on LAN worlds

Turns out setting your mixin environment to "server" bricks it on LAN worlds. The more you know!

Fixed assist credit occasionally not being applied

Fixed player assist credit not being added in some circumstances
Players are no longer removed from the assist table when the entity is dead or dying.

Fixed lootshare.bypassAllowlistTag defaulting to true

Oops haha, should be restricted to the tag by default now.
Also added a README.
