新增任务关注悬浮框

• 会话右侧索引下方显示醒目的蓝色入口，主数字展示任务总数，角标展示待处理与未查看数量。
• 悬浮展开运行中、待处理与已结束未查看会话，离开后收起；点击跳转到已有会话，确认选中后移除本轮提醒。
• 支持拖动记忆位置与一键恢复，避开消息输入框；适配宿主主题。
• 复用状态订阅，不轮询、不加载聊天正文；关闭时不渲染详情，展开时每页最多 20 条。

文档与兼容性

• 中英文 README 补充功能介绍、交互说明与两张模拟会话展示图。
• 要求宿主提供 uiSession.sessionStatus（已核对 DSH 0.2.0-rc.2）；缺少该能力时不显示悬浮框，其余功能可继续使用。
• 提醒来源于当前窗口的宿主状态，不提供独立的跨窗口同步或重启恢复；已结束不代表执行成功。未进行内存占用的定量对比。

• 只自动确保侧栏补丁：并发补丁回到文档里写的手动一步（npm run patch:concurrency），两个宿主对 bash/glob/grep 的调度因此一致。
• 桌面侧栏可以隐藏工作树那一行：设置 → 工作树 的「隐藏工作树行（会话照旧显示）」默认开启——行消失、会话上移到项目的缩进，工作区本身不变；没有活跃会话的工作树保留自己的行。
• 会话悬浮卡片新增面包屑：最上面一行是工作区标题（主工作区 skyrouter，工作树 skyrouter · ed466e1a），下面一行仍是该目录所在的分支。
• README（中英）与 docs/internals*.md 与当前实现对齐；README 里链接到的文件现在随包发布。

Only the sidebar patch is ensured on load again (the concurrency patch stays manual), the desktop sidebar can hide worktree rows, and the session hover card names the workspace above the session title.

• 工作树目录一律使用 $DSH_HOME/worktree/<8 位短码>，不再拿分支名当目录名——同一条分支可以同时挂在多个工作树上，分支名无法保持唯一。
  Worktree directories are always $DSH_HOME/worktree/<8-hex>, never the branch name: one branch can be checked out in several worktrees.
• 会话悬浮卡片跟随会话实际工作的分支：worktree_create 建树时记录，更早的会话从其日志读取一次并写回记录，之后在会话里切分支会自动刷新。
  A session's hover card now follows the branch of the worktree the session works in, recorded when the tool cuts one and read once from the log for sessions that predate it.

• 工作树芯片的会话订阅读了一个未声明的 dir，每次会话更新都会抛 ReferenceError。
  The worktree chip's session subscriber read an undeclared dir and threw on every session update.

• host 215 / client 218 / patch 29 / concurrency 28 / preset 24，全部通过。
