First release of gctl.

• Push markdown files to Ghost as posts or pages, and pull posts back as markdown
• Upload local images, diff, status, lint, new, post open / post delete
• push --watch, push --prune, --json output
• Refuses to overwrite posts edited on Ghost
• Named sites (--site), gctl.toml project defaults, gctl doctor
