Added

• New --pull flag for iscenv start, pulls docker image every time

• Include running iris images in listings of versions

  This allows us to do things like

  iscenv start \
    --image containers.intersystems.com/intersystems/iris-community \
    --version latest-cd \
    example

  Otherwise we can start the first time, but if we iscenv stop and
  then iscenv start it would not find the version.

Fixed

• Switch to getporter/go-plugin fork
  to avoid yamux error message noise.
