v1.4.1: wire premium config into preflight, remove dead config fields

Premium fix:
- account.premium now drives char limit (25k vs 280) in preflight,
  post, reply, thread, analyze, and scheduler commands
- AnalyzeContext gains premium field; analyze display shows correct limit
- Added tests: premium allows >280, non-premium blocks at 280, >25k blocked for all

Dead config cleanup (audited via Codex GPT-5.4):
- Removed account.premium_tier (never branched on)
- Removed keys.bearer_token (codebase uses OAuth1/OAuth2/cookies)
- Removed settings.count (CLI args override, never read)

Underwired fix:
- style.voice now wired into post and analyze AnalyzeContext
  (was only used in agent-info despite comment claiming wider use)

config show now displays Premium status instead of dead bearer_token/count.
