your code looks ok. So your code is not the issue. Probably you are loading the wrong files or so.

I got your emails but I am not offering private support. Despite this, the html you sent would need to be prepared(add missing javascript files, missing custom js-files) so that I cant test it just for the fun of it.

PS: Please dont send me E-Mails if we have this issue-option on github.
