• Fix annotations.readonly vs isReadOnly naming mismatch — tool-registry.js defaulted to isReadOnly but workflow-orchestrator.js checked readonly, causing all tools to be treated as write operations in workflow confirmations
• Fix PHP model ID fallback — Fresh installs would receive Phi-3.5-mini-instruct-q4f16_1-MLC (which no longer exists) instead of Qwen2.5-3B-Instruct-q4f16_1-MLC
• Fix error-log-read.php memory exhaustion — file_get_contents() loaded entire debug.log into memory; replaced with SplFileObject for memory-safe tail reading

Documentation Overhaul

• 46 inaccuracies fixed across README.md, ARCHITECTURE.md, ABILITIES-GUIDE.md, WORKFLOWS-GUIDE.md, and THIRD-PARTY-INTEGRATION.md
• 73 previously undocumented features now documented
• Critical fix: Corrected third-party script handle from wp-agentic-admin-extensions to wp-agentic-admin (wrong handle would cause third-party scripts to silently fail)
