v4.2.3 — Worktree Subdir Fix + Smarter Lenses

• Worktree subdirectory resolution — setup.py computes project_dir (relative path from git root via git rev-parse --show-prefix) and stores it in .evolver.json. Evolve skill Step 0.8 reads this to resolve all worktree paths correctly. Fixes FileNotFoundError when project is in a monorepo subdirectory.
• Smarter uniform-failure lenses — when all examples fail with the same error (e.g., "python: not found"), synthesize_strategy.py now generates a uniform_failure lens (critical) + input_diversity lens instead of only the generic open lens.
