Fixes

• Cross-model continuations no longer fail with 400 missing messages[N].reasoning_content (#334, #337): when a session started on a non-reasoning model continues on a reasoning model, earlier assistant turns have no reasoning to replay. The plugin now sends the placeholder (no reasoning recorded for this earlier turn) for every such turn — text-only and tool-bearing — on both the legacy /alpha/generate transport and the Provider API OpenAI-compatible path. Requests for non-reasoning models are unchanged.
