📌 Summary

This fix addresses an issue where a different tool was mistakenly given the wrong name, causing two distinct tools to appear with the same name.

tools/list returns head_file twice, no tail_file #54

✨ Changes Made

Renamed the file_tail tool (previously mislabeled as head_file)
Added unit tests to prevent duplication in tool names, titles, and descriptions
Updated names and titles for several other tools
