Fix false reauth-required for authenticated ChatGPT account/read responses that include requiresOpenaiAuth: true together with a valid account.

Keep account-less auth signals and explicit authentication error codes fail-closed.

Preserve Codex CLI 0.149.0 App Server compatibility and quota stale-snapshot behavior.

Codex CLI compatibility is fail-closed at 0.144.1; other versions report protocol-mismatch.
Host Surface was verified in isolated temporary profiles with DSH 0.1.0-rc.7 and 0.1.1-rc.2.
Fast Mode is schema-gated and depends on upstream service-tier support. No real model turn was run.
Quota fixture checks pass; live quota remains unverified without a Codex CLI 0.144.1 binary.
