What's Changed

• Work Plan tasks cover only repository work the executor completes and verifies during implementation. Plans no longer add tasks, notes, or caveats for work outside the repository change.
• After the first codebase analysis, the workflow stops once when existing behavior already serves a requirement and the requirement asks only for more. You see the evidence and its cost, and you choose whether to keep or change the requirement before UI Spec, ADR, or Design Doc work begins.
• codebase-analyzer reports differences from requirements as facts and leaves the decision to the orchestrator and designers.
• Requirements are no longer described as "confirmed" or "settled". This lets later phases bring repository evidence back to you instead of treating requirements as fixed.
• The confirmed_requirement_context input is renamed to requirement_context, and the Scope Confirmation section "Confirmed scope" is now "User-selected scope".
