• get-folder-by-id and guarded rename-folder rename a folder in place while
  preserving its identity, notes, and descendants. Renames refuse stale parent
  metadata and sibling name conflicts.
• add-attachment inserts one local file through Notes.app without replacing
  the note body, then verifies prior rich content and the exact fetched bytes.
