Delete & Rename Detection

• All menus now detect when items are deleted or renamed automatically:
  • Functions
  • Custom Menus
  • Commands
  • Regions
• Deleted items are removed from the menu immediately.
• Renamed items are updated in place with their new name.

NOTE: this only works when a list has already been cached.
