Fixes

Tolerate slow configuration probes (#1322)

Allow local Codex and Claude configuration probes up to 60 seconds to
tolerate slow startup. Report timeout fallback and identify probe
cleanup failures while preserving native permission settings and
requiring confirmed cleanup before the model turn starts.
