A conversation is renamed while its turn runs.

Fixes

• Renaming a conversation from its menu while a turn ran (a company conversation in particular) showed session-revision-stale and kept the old name: the turn changes the conversation after every step, and the Gateway refused a rename while busy. The rename now lands mid-turn, the dashboard sends it again against the conversation as it is now, and a rename, fork or archive that still fails says why in words, with the code at the end.

Tests

• tests/managed-rename-web.test.mjs: a company conversation is renamed from its menu while idle and while its turn runs, then with /name, and the name survives a reload.
