• security：/__dsh-outcomes 路由加 loopback + Origin 守卫；该前缀路由不走 Web UI 登录鉴权，
  这样 dsh web 绑到局域网/隧道时，远端不能直接读已登记文件。
• fix：文件路由先 open 成功再 writeHead，并监听 ReadStream error；statSync 与
  createReadStream 之间文件消失时不再产生未捕获异常。
• fix：管理页里的 registry / deletedFile / dir 做 HTML 转义。
• fix：重写临时文件改用 pid + 时间 + 随机 唯一名，降低跨进程碰撞。
• 文档：修正 0.1.1 关于 bin 与 0.1.0 “未 live”的不实描述。
