dsh-statecore 0.4.0

Depends on statecore-mcp@^1.0.0 — the stable 1.x engine (Node's built-in node:sqlite, no install scripts, versioned data files that upgrade in place, Node ≥ 22.13).

• The remember tool declares the engine's new output fields (distillation/reason on the event path, superseded on the note path); previously the strict output schema would have rejected them. A deferred distillation is now reported in the tool's rendered line.
• README: the Prisma postinstall troubleshooting step is gone — 1.0 has no postinstall.
• RELEASING.md updated for the 1.0 publish flow.
