User Story 1

As a case manager, I want submit client-intake information about a client to a tool, so I can better make a decision on what employment services to recommend to a client to maximize their chances of returning to work.

Acceptance Criteria: Clients will be able to input certain data about themselves and receive service recommendations vetted by their social worker.
