Different fixes in the same file are grouped together even if they point to different lines

If we have multiple warning entries with a patch pointing to different locations in the very same file, they appear as alternative fixes for the same issue. This is clearly wrong, only the patches listed explicitly as candidates should be offered as fix options.

The options are not merged together now in the context menu, that is good. However, if I select a fix option I can still iterate over all the patches for the file not just the ones generated for the actual selected issue.

In fact, in the plug-in version v1.0.8 I can now iterate over all the insatnces of the same warning type even if they are in different file, so this is still an open issue.
