Notes:
(1) dm+d drug or device information formerly was used as a SNOMED source and automatically assigned the observation domain, They have been reassigned to the drug domain but in future concepts that default to the observation domain will no longer appear.
(2) + (3) Historic SNOMED concepts that had been ignored before have now been added to deal with older concept codes. These concepts to some degree still have mappings to active concepts, but in many cases will just stay non-standard and invalid.
