add SQL script to generate tables

create a simple script which generates the database and all tables according to appropriate scheme so it's easy to install, and in case we ever migrate to a different server.

added simple script. to run:

mysql -u jackson -p open_encrypt < init_open_encrypt.sql

then in SQL:

SOURCE /full/path/to/init_open_encrypt.sql;
