chore(release): stamp v0.0.15

Cuts v0.0.15 so npm can serve a fresh build. v0.0.14 was published to
npm earlier in the session from a pre-Phase-4 HEAD; npm refuses to
re-publish the same version, so the only way to ship the goal regression
suite, streaming-state labels, two .unwrap() defects fix, and doctor
substrate report to npm users is a forward bump.

Stamped by scripts/bump-version.py v0.0.15:
  - src-rust/Cargo.toml workspace.package.version
  - src-rust/Cargo.lock (12 claurst* entries)
  - npm/package.json
  - README.md (badge + Beta callout)
  - docs/index.md (Version line)
  - docs/installation.md (sample output)
  - src-rust/crates/acp/registry-template/agent.json (version + 5 URLs)

No code changes vs v0.0.14 (HEAD c1368c9). The Phase 4 binary content
was published as v0.0.14 on GitHub Releases; this commit is the npm
distribution catch-up.
