fix: update compatibility tables to reflect actual supported platforms
All four platforms (Claude Desktop, Claude.ai, Claude Code, Claude API)
are supported. IDE extensions listed as future release. Added
ask_user_input_v0 availability note to both Home and Version History.

Adds all Phase 1 documentation pages:
- Home: orientation, phase scope, page index
- Architecture: engine design, file structure, skill-widget contract
- Sequence Diagram: normal flow and self-test flow
- QUESTIONS Schema: field definitions, rules, annotated example, payload format
- Integration Walkthrough: 4-step guide, Claude Desktop install, minimal example
- Self-Test: trigger phrases, pass conditions, troubleshooting table
- Common Mistakes: 5 named failure modes
- Version History: compatibility table, full changelog v1.0.0 through v4.0.0
Phase 1 scope: Claude Desktop for Mac. Tested on 1.12603.1 (3df4fd), 2026-06-11.
