Remove root entry point generation

Description

As mentioned here, we do not need to generate the root entry points for this library.

AC

• Set rootEntrypoints: false in the loom.config.ts file
• Remove index* references in the various ignore files (e.g. .gitignore, .eslintignore, and .prettierignore)
