Fixes an issue where the hidden input would not be renumbered. `.fm-checkbox-hidden` and `.fm-element` are analogous to the classes assigned to the hidden Autocomplete field.

.fm-checkbox-hiddenand.fm-elementare analogous to the classes assigned to the hidden Autocomplete field.Also addresses a related bug with checkboxes in repeatable fields where the hidden element keeps the
[proto]name attribute when the field is added.
