Expiry detection heuristics

Accounts are classified as expired when:

• Token refresh fails (session expired, token revoked, token conflict)
• Auth fails (HTTP 401/403 after refresh retry)
• Plan type is free (subscription lapsed)

Transient errors (network timeouts, server errors) are not treated as expiry.
