MCP resources refresh

Currently Crush is the only Agent harness which i use, that support Tool & Prompts refreshing following the spec.

However the Resources is missing.

I did build the feature, i just and tested it.

I just need approval from a maintainer to open a PR.

I think I have a PR or a branch for that as well - @kujtimiihoxha we need to properly discuss attachments and how we want to make MCP resources available.
