Configurable MCP init timeout

Hi, many mcp tools use e.g. npx in their launch instructions. Unfortunately tools like npx can be slow due to downloading all deps.

Suggest adding config option to set mcp_timeout to override the default 10 second waiting time. e.g.

        "context_paths": [
            "AGENTS.md"
        ],
        "debug": true,
        "debug_lsp": true,
        "mcp_timeout": 20
    },
